Create a Meaningful Obituary

Obituaries are as unique as your family.  We have created this online form to assist you with this process. Please fill in the family members that you would like to include in the obituary in the form below.  You will also be asked for some other pertinent information, which will help us create an obituary.

Key tips:

  • the form builds the obituary just like a family tree
  • focus your efforts on proper spelling and ensuring that everyone that should be included is included
  • you will have the opportunity to customize and personalize the obituary at our arrangement conference
  • you will receive an email including all of the information you submit immediately after completing this form
  • ***Please include any family members that have predeceased your loved one with a (D) before their name.  For example: John Doe of Crediton, (D) Mike and Sarah Doe of London, and (D) Jane Smith

  • It is a family decision whether or not you include in-laws in the obituary.

  • Sometimes families wish to include:
    Community, Church, Service Club, Sports involvement, Career Recognition, and/or Hobbies.
  • We would encourage you to be as specific as possible, so the funds collected for your preferred charity will make a difference where you intend.

    An example that demonstrates this is "Breast Cancer", which has many different affiliations in Ontario (this is an example only and not a recommendation for your family).
